The MI Medicaid CHW Registry is a database of Community Health Workers (CHWs) who have met qualification criteria required to provide CHW services to Medicaid beneficiaries in Michigan. Individuals can become MI Medicaid certified CHWs through the CHW Training Program.
Before registration with the Community Health Automated Medicaid Processing System (CHAMPS) for Michigan Medicaid reimbursement, a CHW must be verified and added to the MI Medicaid CHW Registry. MiCHWA, in partnership with Michigan Primary Care Association, is the designated vendor to provide verification and credentialing services.

Step-by-Step Instructions for Completing an Application to the MI Medicaid CHW Registry:
Step 1: Acquire a National Provider Identifier (NPI)
You must have a (Type 1) National Provider Identifier (NPI) as a Rendering/Servicing-Only Provider to complete your application through the MI Medicaid CHW Registry (instructions can be found on the National Plan & Provider Enumeration System webpage.
Step 2: Gather Necessary Documents and Information
Before starting, ensure you have all the necessary information available to complete the MI Medicaid CHW Registry Application. Here is a list of the information and documents will need to have:

- Full Name
- Date of birth
- Contact information and mailing address
- Social Security Number
- Year High School diploma or GED completed
- Individual (Type 1) National Provider Identifier (NPI) as a Rendering/Servicing-Only Provider (See instructions above in Step I)
- Training program information (date of completion, name of training program provider)
Step 3: Complete the Application
Go to the MI Community Health Worker Medicaid Registry online application and click on “Start Now” to complete your application.

The link to the application is available beginning January 1, 2024.
Step 4: Submit ALL Verification documents to complete your Application
When you have finished your MI Community Health Work Medicaid Registry Online Application from Step 3, next send all verification documents. If you are applying under the CHW Training Program Pathway, email your certificate of completion. transcript or other proofs of course completion documents.

Applications can be submitted at any time throughout the year. All applications are reviewed by a CHW Training Program Review Committee. The CHW Training Program Review Committee is a community-led, inclusive volunteer workgroup, composed of CHWs, CHW Stakeholders and/or CHW Allies. The role of the Review Committee is to review all CHW training program applications to ensure minimum requirements outlined by the MDHHS Medicaid Policy are met and issue a final determination.
The review and scoring process will align with the National Core Competency Standards (C3) and Michigan Medicaid policy requirements. This ensures that the approved training programs adhere to the highest standards, providing CHWs with the necessary skills and knowledge to excel in their roles.
In fostering a transparent and impartial evaluation process, it is important to note that MiCHWA staff does not directly engage in the scoring of applications; instead, they serve to support and facilitate the efforts of the Review Committee.
