PTI’s Integrating Community Health Workers into Palliative Care Micro-Credential builds on knowledge from certification training. The 2-day program provides CHWs with skills, knowledge, and confidence to effectively support individuals and families living with serious illnesses. Training includes serious illness and symptom burden; defining palliative care and differentiating palliative care and hospice; palliative care domains and how these impact patients and families; CHW core competencies and roles contributing to effective palliative care; CHWs as members of the palliative care team; effective communication with patients and families during stressful periods; cultural considerations; and care at the end of life.
